delivers comprehensive health care services through four clinics, multiple
mobile outreaches and public-private partnerships within Kampala, Wakiso,
Nakaseke and Luweero.

Job Summary:   The Linkage to Facility Officer will provide
technical support to the program implementing CSO and linkage facilitator to
ensure effective program implementation and compliance to project, MGLSD and
USAID guidelines. He/she will ensure effective application and implementation
of the program case management procedures to ensure that the needs of program
beneficiaries and vulnerability risks within their households are adequately
addressed. The incumbent will actively participate in the development and
implementation of core program activities and implementation processes that
will facilitate linkage between and within communities and health facilities.
He/She will provide regular technical and methodological soundness of
activities implemented by CSOs and Para Social Workers that are part of the
project implementation. 

Key Duties and Responsibilities: 

  • Provide technical guidance to program Social Workers and Linkage
    facilitators for establishment of effective and functional
    community-facility OVC services referral networks
  • Support CSO Social workers to plan and conduct monthly case
    conference meetings with Para Social Workers to review and assess progress
    on implementation of OVC case plans and performance of HIV/AIDS referrals
    from community to facility and facility to community.
  • Offer timely technical assistance to Social workers and Para Social
    Workers on OVC HIV case identification, linkages to care and Treatment as
    well as Retention strategies
  • Support the process for identification, training and coordination of
    linkage facilitators to improve on proportion of referrals made and
    completed.
  • Support implementing CSO to plan and conduct quarterly referral
    reflection meetings to assess progress on services and referral
  • Participate in district and community level service provider
    linkages and services coordination meetings
  • Carry out capacity gap and training needs assessment for CSO social
    workers, referral facilitators and Para Social Workers and design
    appropriate training activities on case management, HIV/AIDS sensitive
    child protection and referral coordination
  • Support the development and facilitation of skill based case
    management training for Para Social Workers and CSO social workers using
    effective, relevant and appropriate delivery mechanisms
  • Facilitate learning among OVC partner project implementing CSOs, LG
    staff on effective and need based individual and household case management
    and referral systems
  • Take part in district and community level service provider linkages
    and services coordination meetings for learning, information and
    experience sharing.
  • Support CSO, Sub County and Linkage Facilitators to coordinate and
    participate in integrated quarterly joint support supervision activities
  • Document best practices at community level and within the referral
    and linkage system in form of case studies, success stories and abstracts;
    and share at designated forums at sub county level, project and district
    level

Qualifications, Skills and Experience: 

  • The applicant must hold a Bachelor’s degree in any of the following
    fields: Social sciences, Education Community Development or related
    discipline. Project Planning & Management shall be an added advantage.
  • Three years of professional experience in a similar or related job
    gained through working on any OVC or public health program
  • Previous exposure, experience and ability to coordinate activities
    of different stakeholders at district and community level,
  • Excellent report writing skills
  • Demonstrated ability to work effectively both independently and or
    in a team environment. Excellent interpersonal skills
  • Strong planning and coordination skills
  • Broad knowledge of Nutrition and food safety, HIV/AIDS care and
    prevention, OVC education coordination and support.
  • Excellent analytical and problem solving skills
  • Excellent computer, verbal and written communication skills.
  • Ability to work under pressure to tight reporting deadlines.
  • Ability and willingness to travel 60% of the time
  • A non-judgmental approach regarding race, religion, culture,
    sexuality and lifestyle is critical, Fluency in both English and Luganda
  • Ability to ride Motorcycles with a valid riding permit
