Diseases Institute (IDI) is a Ugandan not-for-profit organization whose mission
is to strengthen health systems in Africa, with strong emphasis on infectious
diseases, through research and capacity development. IDI has six operational
areas through which it works to achieve its mission: Prevention, Care and
Treatment; Training, Research, Laboratory services (including the CAP-certified
IDI Core Lab & Translational Lab), Global Health Security and Health System
Strengthening.

 

Job Summary:  The Community Liaison Officer will take lead
in implementation of the community activities for the pediatric and adolescent
HIV project. S/he will support the establishment and maintenance of functional
facility-community linkages with the aim of improving identification, linkage
and retention of pediatric and adolescent clients in care through
community-based initiatives.

 

Key Duties and Responsibilities:  

·       
Support
identification, linkage and retention of all identified HIV Positive children
and adolescents.

·       
Support
documentation of clients using the family tracking tool and client locator
forms.

·       
Follow
up children through phone call or home visit for linkage, initiation and
retention.

·       
Provide
home-based care and support for HIV positive paediatric and adolescent clients.

·       
Follow
up PMTCT mothers for ART initiation, missed appointment and linkage to EID
services.

·       
Work
with Village Health Teams to find vulnerable children in the community that are
at increased risk of having HIV.

·       
Plan and
coordinate targeted outreaches to these children.

·       
Follow
up difficult adolescents who have missed appointments, have drug adherence
issues or are lost to follow up.

·       
Organise,
coordinate and supervise community-based expert clients involved in physical
tracing of lost clients.

·       
Support
established Community Based Organisations and PLHA networks to identify, link
and retain children and adolescents in care.

·       
Mobilize
and support regular community-based meetings of special client categories such
as adolescents and young mothers such as post-test clubs and caregiver
meetings.

·       
Compile
monthly reports to track progress of activities.

·       
Ensure
linkage of clients to community support structures for nutrition, economic
empowerment for holistic care.

·       
Ensure
proper facility-community linkages to cater for the non-medical needs of
caregivers, children and adolescents.

 

Qualifications, Skills and Experience:

·       
The applicant
must hold a degree in Social Work and Social Administration, Community
Psychology or Developmental Studies

·       
Experience
in counselling and working with children

·       
Experience
in community-based health care initiatives and HIV-related support services is
an added advantage.

·       
Understands
community dynamism and engagement.

·       
Good
advocacy for HIV/AIDS

·       
Good
communication, organization and management skills

·       
Understands
client confidentiality and exhibits high level of ethical conduct

·       
Has
basic computer literacy including word processing, excel, internet and power
point.
