Regional Health Integration to Enhance Services – North, Lango Project
(RHITES-N, Lango) is operating in the nine districts of the Lango sub-region of
northern Uganda (Alebtong, Amolatar, Apac, Dokolo, Kole, Kwania, Lira, Otuke
and Oyam). RHITES-N, Lango will increase the effective use of sustainable
health services in the region. The Project is led by John Snow, Inc. in
partnership with African Medical and Research Foundation (Amref), Doctors with
Africa (CUAMM), The Medical Concierge Group (TMCG) and Another Option. The
project will contribute to measurable improvements in key Ugandan national
health indicators (HIV; tuberculosis [TB]; malaria, maternal, newborn, and
child health [MNCH]; family planning [FP]; water, sanitation, and hygiene
[WASH]; and nutrition). The USAID RHITES N, Lango Project, in partnership with
Ministry of Health, Lira Regional Referral Hospital (LRRH) wishes to
re-advertise for the following positions that were run in the New Vision
Newspaper of March 11, 2019. USAID RHITES-N, Lango will facilitate the
recruitment of qualified Ugandans on behalf of Lira RRH. The hospital is a 400
bed hospital situated in Lira Municipality in Lira town with a catchment
population of about 2.3 million people. It is a referral facility offering both
specialized and general services.
Job Summary:  The Linkage and Referrals Officer will oversee
implementation of the intra-facility, inter-facility and bi-direction
community-facility linkages and referrals component at the hospital. The
position entails support to VHT activities, Linkage facilitators, Male action
groups (MAG), Family support groups (FSG), MNCH peer mothers, adolescent health
champions, differentiated service delivery, improving community referrals and
reporting for health and nutrition services. The officer will ensure timely
reporting, proper documentation and updates of clients’ information in the HIV
clinic, antenatal care/ Mother baby care points and Tuberculosis units.
Key Duties and Responsibilities: 
  • Coordinate linkages for people living with HIV/AIDS, TB clients and
    other clients identified by the hospital
  • Support the functionality of CHEWS, VHTs, MNCH Peer Mothers,
    Adolescent champions, Male action groups and Family support groups within
    the hospital and community served
  • Strengthen client appointment and tracking systems at the supported
    health facilities; including contact tracing for TB and HIV
  • Strengthen quality improvement structures at community levels to
    improve on the referral and linkage system and other community activities;
  • With the hospital teams, support planning for and implementation of
    integrated outreaches
  • Support a bi-directional linkage and referral system of clients from
    community to facility, and strengthen the implementation of community
    referral systems
  • Strengthen client appointment and tracking systems at the supported
    health facilities; including contact tracing for TB and HIV.
  • Work with hospital staff to document and update all community
    component related registers.
  • Support community based structures to deliver Health, WASH and
    Nutrition education messages to the target beneficiaries
  • Train and work with community resourceful persons including, VHTs,
    Sub/County Health Assistants, and local leaders to strengthen VHT
    reporting and use of community tools.
  • In collaboration with the clinical and community teams provide
    data  on community interventions to
    ensure timely reporting, including the District Health Information System
    (DHIS2).
  • Coordinate activities for demand creation for all supported health
    facilities, through conducting feedback meetings, community dialogues and
    FSG meetings.
  • Ensure proper documentation of work done, achievements registered,
    lessons learnt, throughout the implementation and scale up best practices.
  • Support and participate in extraction and generation of lists of
    clients (including but not limited to missed appointments, lost and lost
    to follow up clients, clients due for viral load tests and non-suppressed
    clients): share the list with responsible persons and structures for
    tracing and return back to care.
  • Coordination of peer support activities.
  • Perform any other duties assigned by the supervisor



Qualifications, Skills and Experience: 
  • The applicant must hold a Bachelor’s Degree in Sociology, Social
    Work and Social Administration, Community Development, Community Health,
    Development Studies or related courses.
  • Postgraduate qualification in Public Health, Sociology or Community
    Health would be an added advantage.
  • Three years of experience in HIV/TB/MNH/Malaria/Nutrition related
    community work with government institution or NGOs involved in community
    work
  • Skills in strengthening health and nutrition service delivery
    programs, supporting referrals and linkages between and among communities
    and health facilities
  • Excellent communication skills and strong inter-personal skills
  • Fluency in written and spoken English and must be able to
    communicate in the local language (Lango)
  • Ability to work in a multi-disciplinary team with frequent travel to
    the field
  • Have solid computer skills.
  • Knowledge of the relevant Uganda national health policies,
    strategies and structures
  • Must be respected in the community and recommended by LC1 Chairman
