Health Integration to Enhance Health Services in East Central Uganda Project
(USAID/Uganda RHITES-EC) whose aim is to support the implementation of a
comprehensive integrated investment in regional
health, nutritional assistance and HIV/AIDS programs. The five-year
project targets 11 districts in East Central Uganda with an estimated total
population of 4.1 million through a district-based integrated package of
quality health, nutrition, HIV / AIDS and child development services.
Facility Linkages Coordinator is primarily responsible for working with the
hospital management to contribute to improved linkages along the continuum of
care and improved retention of all HIV positive clients at RHITES supported
hospitals. Plan and coordinate community linkages and referral activities in
collaboration with Hospital Health Management Team and other stakeholders/partners
to ensure successful implementation and monitoring of the project. He/she will
coordinate activities to support community-facility, facility-community and
intra/inter facility community linkages
Work closely with Hospital health teams to
establish community based structures that will create demand for HIV/AIDS care,
treatment, Nutrition, SMC, MNCH, malaria and PMTCT and TB/HIV services
Supervise Community Health Workers (CHWs) to
conduct referrals, follow ups and other community activities
Coordinate and monitor the referral system
between the community and the Hospital
Offer technical support to Community Health
Workers (CHWs) to strengthen community-facility linkages aimed at improving
patient adherence, retention, self-management, and psychosocial services.
The jobholder will also coordinate the
implementation of community mobilization approaches, documenting best practices
for community engagement, linkages and HIV prevention at community level
Work with community M&E officer and health
information officers to coordinate and monitor community linkage activities and
performance.
Work closely with Hospital health teams and
volunteers to establish new and strengthen existing community treatment support
groups to promote drug adherence, retention
The Community Facility Linkages Coordinator will
prepare a comprehensive monthly and quarterly reports of all activities carried
out as per the work plan
Perform any other duties assigned by the
supervisor

Experience:
The applicant for the USAID RHITES-EC Community
Facility Linkages Coordinator job opportunity must hold a Bachelor’s Degree or
higher qualification in any of the following disciplines; social sciences,
Social works, Arts , Health Management, Development studies, Community
Development or other relevant field.
At least three years of related experience in
community health, preferably with USAID funded projects, community health
service delivery and experience at community level health systems in Uganda.
Extensive knowledge of latest developments in
areas of pediatric, adolescent and adult HIV counseling.
Excellent written and verbal communication or
interpersonal skills is a must.
Good analytical skills and ability to support a
cross section of Community health workers and volunteers
Ability to express ideas clearly in written or
oral form plus proven skills in computer, mentoring and training supervisees
