aimed at reducing the HIV incidence, and mortality related to HIV and TB and
develop capacity for the region and the country to plan, coordinate and sustain
HIV epidemic control. The program will accelerate Epidemic Control in Mubende
Region under the President’s Emergency Plan for AIDS Relief (PEPFAR). In
partnership with Mildmay Uganda and support from CDC, PACE’s primary role will
be to PACE’s to strengthen community systems to yield high HIV case finding,
linkages and community based support for retention in HIV care and prevention.
finding & Linkages Officer will be responsible for case findings and
leading the referral processes for HIV services. The incumbent will also
implement the project activities in line with the donor requirements, PACE’s
vision and mission.
Responsibilities:
- Render support in the mapping of
facilities and rollout community framework for HIV integrated service
delivery. - Support the district community structures
and CBOs to spear head planning, coordination, monitoring, supervision and
implementation of linkages and case management - Formulate and coordinate network referral
channels, plan to increased linkage and referral of PLHIV into treatment - Implement proper usage and documentation
of data on linkage to treatment and other integrated services - Carry out proper documentation and
tracking of PLHIV for linkage, access and retention in care - Provide support in the collaboration
between the community structures such as village health teams (VHTs) and
the CBOs. - Implement and follow up facility and
community systems to improve linkage of identified PLHIV to HIV care &
treatment services. - Implement Differentiated Service Delivery
models for stable clients (ARV refill, Client led ART delivery, Community
Drug distribution points, Community Pharmacy) - Mobilize and link Community systems to
improve adherence and retention in care - Mobilize, refer and link TB support
structures for community TB screening, contact tracing, referral, tracking
& follow-up
Experience:
- The ideal candidate for the PEPFAR Case
finding & Linkages Officer career placement should hold a Bachelor’s degree in Social Sciences,
Community Development, Community Health or Nursing. - Possession
of a post graduate qualification in Public Health, Sociology or Community
Health would be an added advantage - A minimum of three years’ experience in community work particularly,
working with local governments; and ability to engage DHTs and lower level
community structures. - Precious experience in building capacity at individual and organizational
levels - Comprehensive
knowledge of the decentralized health system and the corresponding
community structures. - Excellent
communication, interpersonal and customer service skills - Good
