Sustainable Outcomes for Children and Youth (SOCY) Project
non-governmental, non-profit, non-political, inter-denominational charitable
organization that was established in 1999 by a group of volunteers as a
response to a crisis caused by HIV &AIDS and poverty in rural communities
in Uganda. FOC-REV Ministries Uganda exists to work with the poor and
marginalized rural people to improve their livelihood and increase self-reliance
through education support, health services advocacy, food security and
networking.
implementing a Sustainable Outcomes for Children and Youth (SOCY) project targeting
Orphans and Vulnerable Children in Hoima and Luwero districts, funded by the
United States Agency for International Development (USAID) through Catholic
Relief Services (CRS). The SOCY project interventions shall be focusing
primarily on OVC Case Management and household economic strengthening through
savings groups and associated interventions and parenting/caregiver programs.
and Referral Coordinator will play a pivotal role in developing and ensuring
functional bi-directional referral systems between communities, clinical
services and social services Providers.
Responsibilities:
- Oversee the establishment and or
strengthening of community OVC referral and service coordination networks
in the target communities. - Regularly track and monitor the success of
the referral systems, and provide regular technical -support to community
and facility based referral facilitators or volunteers. - Work in liaison with other regional or
District based USAID funded HIV/AIDS partners to support and facilitate
beneficiary HIV Testing and Counseling, linkage to care and - Plan and conduct quarterly Sub county and
district level referral reflection and service provider coordination
meetings with other key bidirectional referrals stakeholders. - Coordinate with Social Workers and other
USAID IPs to plan and conduct targeted OVC service camps in hard to reach
program communities - Prepare a referral progress report on a
monthly basis and conduct monthly case conferences with community volunteers - Utilize program data to track beneficiary
linkage into HIV/AIDS care and adherence
Experience:
- The ideal candidate for the USAID Project
Linkages and Referral Coordinator should preferably hold a Bachelor’s
degree in Social Work, public health, social sciences or related
qualification - Possession of a Post graduate training in
child protection methodologies and best practices - At least two years’ experience
implementing linkage and Referral initiatives with government and/or USAID
funded projects - Previous experience/knowledge of case
management identification and assessments, planning, referral and support
services, monitoring, review and closure tools - Excellent interpersonal and communication
skills - Ability to work independently, multi-task
and take initiative. - Strong leadership capabilities and
demonstrated potential for learning new skills - Familiarity with OVC- and HIV-related
programming - Hold a valid Motorcycle Riding permit
